It appears to be the same failure I had.
I'm guessing the EGR pip manufacture quality control wasn't very good on the early TDv8 ...

An 015 size is on the small size gor an L322.
Best is an 020 - but doesn't fit all.
Or slightly shorter is the recommended size which is the 019
