Sounds like your procedures on forklift running should be updated or the driver(s) need additional training. If the forklift driver can't see because the bucket is too big then no more use of the bucket unless there is a spotter guiding the driver. If the bucket can be lowered for proper viewing then the driver needs a big slap on the wrist. Imagine if it would have hurt someone, you would have had some big issue with the HSE lot. Legally you are responsible and could end up with major issues that could make a company go under financially.

I agree in principle. I also work in O&G, obviously you wouldn't find a RR offshore to drive a forklift into but safety is drilled into us every day. I guess it's a lot different working within a family business though and I'm sure lessons will be learnt. Again thankfully it's only metal that is damaged (and reputations ha).
