Fitting towball

I might be missing something here but how do you fit the towball into the receiver?
I have lubricated all the bits and everything is free but I can't get the towball to fully engage with the receiver(?). I turn the spring loaded handle fully and the 3 ball bearings partially retract but not fully, consequently the towball wont fully engage. Am I missing something obvious?
Also what is the purpose of the cam type lever on the front of the towball?
I assume if I ever get the two bits to engage the idea is to lock the handle and remove the key? My key is only removeable in one position, is that correct?
Anybody happen to have a set of instructions?

With the key in or out the result is the same.
Also the key doesn't seem to lock anything or could it be the towball has to be engaged before this happens?

The wheel has to be 'cocked' before it will engage. Turn it round until it locks in place, then push into the receiver. I can't remember if you have to pull it out at the same time as turning it but no doubt someone else will remember.
