That has been worked on before.
They have cut the top of the fan shroud.
Was tempted to do the same myself when I need to remove fan again.

Interesting, I was convinced that was a video I shot until I saw your name at the bottom. I have a 2011 TDV8 4.4 making exactly the same noise. Someone has even cut off the top of the fan shroud exactly as has been done on yours.

I assumed it was a lifter problem but I've not managed to look in to it yet.

What do you think is wrong with the piston that would make a tapping noise rather than a terminal rattle?

Based my opinion on experience in commercial vehicle repair, where we have experienced similar “tap” which often turns out to be piston related. It’s possible that the piston has seized and the skirt has picked up. Seized due to a local overheat. Maybe oil cooling jet to a particular cylinder blocked or misaligned - if this engine has them. Have also known vehicles to run low on coolant and the same thing happen to the hottest cylinder ie no 4 or 5 on a straight 6.
This is of course an armchair diagnosis/opinion and could be completely wrong and have been! It would be nice to learn the outcome, but hopefully the vehicle will be returned for a refund, so this will be unlikely.
Let’s hope if it’s returned a decent repair is carried out before this vehicle is put back up for sale.

Timing chain jumped a tooth and valve 'kissing' top of piston?

Any diesel smell in the new oil?

Have you tried disconnecting the injectors one by one on that side to se if the noise stops?

Not necessarily, happened to me on a bmw 325tdse, everyone thought it was an injector off, only became apparent when the head was off, didn't even bend the valve stem and just a small depression in the top of the piston Cool
