Due to a bit of drill left in a root I may need one of these

Well it all seems a bit extreme (dentist here love you to spend lots of money so i always check around and invariably some one provides a more suitable offering)

I had root canal a few years ago and its never been great. Often feels raised also has a few 'blisters' on gum outside.
Had a 3d scan done and one root of the lower molar(one behind pre molar) has heavy shading around it. This also identified a piece off a tool on another root same tooth..though that one doesnt have this infection around it.

What I would like to know is.
1) I asked about antibiotics..they said no use...true or false
2) I asked about removing crown and re do roots suffing a bit medicine directly down(not root with tool in.) They think some fibre substance has been put in roots which they could not drill through..true or false

3) any other options...removal..bridge..

Just after more information. If the Apicectomy is required I would rather fly back to have it done or some other country and by someone with experience of this but have no idea of the best place to do it.

Right ok.

1. Antibiotics - I agree, no use. They can be used to reduce the infection but they will not address the source so it will return fairly quickly.
2. Your thinking seems correct - ideally, the root which has the residual infection would be re-treated. The success rate of this is variable dependent on many factors. If there is a fibre post bonded in to this root, it can be very difficult (or risky) to try and remove. This post would have to be removed first to allow access to the root canal.

Whilst the other root has a broken fragment of instrument in it, if there is no evidence of infection around this root, it is not necessary to treat this. Many roots can be left with bits of instruments in them for many years without issue. I'd focus my attention on the root with the infection - however, this may be the one they want to do the apicectomy on.

Extraction of the tooth is a sensible option: both apicectomies and re-root canal treatment do not have fantastic success rates. You may be better cutting your losses, extracting the tooth then considering options to restore the space - bridge/ implant/ part-denture.

I hope that is of some help. I've probably just backed up what you were thinking, but there's no harm in that.

I cant get back to the uk to get it yanked for a while( read tax reasons) I will go for a re root job as an interim try. If that fails will go with removal when I can get back.
