Juddering and cold start issue

Hi I am getting a judder between 30-50mph on my 02 td6, I have looked through the history and can't find any evidence the gearbox oil has been changed. I'm now approaching 145k miles, is it worth changing the gearbox oil or should I just have a new torque converter fitted ?
Also when I start it from cold it runs quite lumpy for a few minutes, could this be an injector problem which could also be causing the judder as above ? Guess it's getting to that time when things start going wrong, would like to keep the range so need to get these problems sorted sooner rather than later before more damage is caused. Any help or advice would be greatly appreciated

Change the gearbox oil - see how it goes, if the TC is "gone" likely the gearbox won't be far behind - it has done well if it is the original at 145K (but I doubt it, normally the gearbox lasts @ 80K)

Does the car start immediately on turnover or does it take a few turns to start up? If the latter this normally points to injectors. You will need to do a leak test to identify which injectors are causing the problem, then replace them (easier said than done) .

My gearbox went at 137k which is a good innings. When mine went it was pretty sudden with a bit of juddering and then into limp mode.

FYI You should always change the gearbox oil every 60k to keep an eye on any break up in the TC. Tom
