Coolant cap issue ?

Did anyone ever had a problem with coolant cap not holding pressure ? I had a big leak at top back engine, replaced parts, and no pressure in the system. Initially thought that it's a water pump, replaced that, still no pressure, but when you put the
measuring tool in the tank the pressure is on, heating work, all seems to be fine till the tool poops out. Could this be a simple cap issue ? Also the dash shows warning coolant level low.
