I read David Jason's autobiography recently and he describes in detail how the sketch came about. It was an incident that John Sullivan had seen, but it was David Jason's idea to fall like a log - which is not that easy to do.

Trigger's reaction is brilliant. That wall like stupidity.

And also that Trigger (Roger Lloyd-Pack) wasnt even scheduled to be in that scene, he was filming another series at the time and just happened to wander into the Only Fools studio!
